Biography: Following residency, he was in individual and small group practice part time, while teaching at the Northridge Residency part time. He joined the Memorial Family Medicine faculty in 1997 and assumed the residency director position in 2001. Dr. Bauer enjoys the full spectrum of family medicine and has a strong interest in the use of technology to enhance both resident education and patient care. In his free time, he enjoys woodworking and magic, as well as spending time with his wife, Leslie, and two boys, Nathan and Gavin as well as woodworking & magic.

Jamie Cazares, M.D.

Biography: Dr. Cazares worked at several residencies in Illinois and Texas prior to joining the Memorial Family Practice Residency Program in 2003. While in Chicago, he completed a faculty development program at Cook County Hospital Family Practice Fellowship Program. He gained additional research experience at The University of Chicago in the Diabetes Research Training Center (DRTC). In addition, he continues to maintain an interest in Traditional Chinese Medicine. Moreover he has training in acupuncture and herbal medicine. Dr. Cazares had been active with the American Academy of Family Physicians in the following capacities: served on the Minority Health Affairs Committee, appointed to the AAFP Commission on Education, elected as one of the first Minority Physician Delegates and selected as the Convener for the Women Minority and New Physician Conference. He is married, has two daughters and is fluent in Spanish. In his spare time, he enjoys outdoor sports and Qigong.

Biography: Dr. Chang grew up in west Houston, attended the Wharton School at University of Pennsylvania, and received his medical degree at UTMB in Galveston.  He completed his residency at Memorial Family Medicine Residency Program where he also served as one of the Chief Residents. He then did his sports medicine fellowship at Bayfront Medical Center where he served as team physician for the University of South Florida Bulls in Tampa, Eckerd College Tritons in St. Petersburg, local high schools, as well as took care of marathoners, triathletes, workers, musicians, and dancers of all ages.  He currently serves as team physician for the Houston Baptist University Huskies and area high schools in Fort Bend county.  He enjoys the full spectrum of family medicine and loves to travel, hang out, snowboard, road cycle, and play pick-up basketball and club volleyball in his free time.

Biography: Born and reared a Hoosier, Dr. “Dallas” has always been accused of being a Texan, and since 1994 he has been one. Dr. Coate practiced in a small town for 14 years and then completed a 12 month obstetric fellowship. He has been in academics since 1994 teaching at Memorial Family Medicine Residency and the Brazos Family Practice Residency.

Biography: Dr. Crouch sees patients 30% time and is the research director for the residency program. Following residency training and fellowship, he completed a two-year training program in family therapy through the Menninger Clinic. Before coming to Physicians at Sugar Creek, he was an associate professor of Family and Community Medicine at Baylor College of Medicine in Houston for 18 years. His clinical interests include cholesterol, diabetes, heart disease prevention, obesity, anxiety, depression, and attention deficit disorder.  Dr. Crouch's research interests include shared decision making for statin therapy to prevent coronary artery disease, the early detection of diabetes and pre-diabetes, and the effect of low literacy on patient education.  Dr. Crouch enjoys spending time with his wife Cassey, and visiting his son Andy and daughter Cara in Austin. He also plays guitar, sings, and writes songs for a church band, The Yellow Dog Howlers, runs and bicycles, and does genealogy.

Victoria Tu Do, M.D.

Biography: Dr. Do received her medical degree from St. George's Medical School, London, United Kingdom. She completed her internship in England and her residency in Family Medicine at Memorial Family Medicine Residency Program. She served as the inpatient hospitalist at Memorial Hermann Fort Bend Hospital and joined as faculty in 2004. Dr. Do is interested in evidence-based medicine, women's health and preventive care. Her philosophy in the training of residents is directed at a warm and approachable faculty/resident relationship, encouraging questions and discussion so that each graduating resident will become an excellent family physician. Dr. Do is fluent in Vietnamese. She enjoys spending time with her husband, daughter Olivia and Son Mark.

Troy Fiesinger, M.D.

Biography: After completing residency in 1999, Dr. Fiesinger practiced family medicine including obstetrics with the Scott and White Clinic in Waco, Texas for seven years.  In 2006, he joined the faculty of the Conroe Family Medicine Residency.  He also served as the clinic medical director for the Lone Star Family Health Center, a federally qualified health center, at that time.  He joined the faculty at the Memorial Family Medicine Residency in 2009.  Dr. Fiesinger practices “full spectrum” family medicine including obstetrics and office procedures.  He has particular interests in practice management, evidence-based medicine, and quality improvement.  He also actively participates on committees and commissions of the TMA, TAFP, and AAFP.  Dr. Fiesinger and his wife, Ellen, a family nurse practitioner, have two children, Leo and Emma.  His hobbies include marathon and ultramarathon running, road and off-road cycling, and a new affliction – golf.

Geraldine C. Gossard, M.D.

Hometown: Charlestown, Ireland

Biography: Dr. Gossard came to the United States from Ireland. She graduated from the University College Galway School of Medicine in 1982. She completed rotating residency in family medicine in Ireland and England. She joined Memorial Family Medicine Residency Program as a resident in 1993 and stayed on as assistant professor of the Department of Family and Community Medicine and faculty at the program. Dr. Gossard's interests include women's health, prevention and geriatrics. She has a strong interest in evidence-based medicine and research and has presented at national conferences. Dr. Gossard and her husband, Carl, have two young sons and, when not at the ballpark, she enjoys spending time with family, cooking, travel and golf.

Biography: After serving with the United States Air Force for four years and obtaining his undergraduate degree at Houston Baptist University, Dr. Howard attended the University of Texas Medical Branch in Galveston where he received his medical degree. His Family Medicine residency training was completed at Baylor College of Medicine Affiliated Hospitals Residency Program in Houston . Dr. Howard has worked with the Memorial Family Medicine clinics for a number of years, joining the faculty in October 2003.

Biography: As a former National Health Service Corps scholar, he has enjoyed practicing over 12 years in the field of community medicine working community health centers in Wisconsin and Georgia. He has strong interests in medical education and practice management and enjoys infusing the esoterics of clinical teaching with the applied practicality of management theory. He has been the medical director of several practices, most recently the Morehouse Family Medicine Residency’s health center in Atlanta, GA. He is also an active Reservist in the US Army and has been deployed numerous times in support of the nation’s wartime operations. In his spare time, he enjoys all kinds of sports, national politics and discussing health care reform. He also loves to spend time with his two children, Quentin and Kiersten, and wife, Angela.

Biography: Dr. Moquist is board certified in Family Medicine and holds a CAQ in Geriatrics. He also has a CMD for Long Term Care. He has been active in organized medicine. He has served on the Board of Directors of the American Academy of Family Physicians and is presently Chair of the AAFP American Medical Association Delegation. He has served on the Commission on CME for the Texas Academy of Family Physicians and is presently President of the TAFP Foundation. After being in private practice for over 20 years and working with other residency programs, Dr. Moquist joined the Memorial Family Medicine Resident in August 2004. He coordinates the geriatric curriculum which includes the month-long geriatric rotation, home visits, monthly conferences, and admissions to a Medicare Skilled Unit.

Wilson R. Moscoso, M.D.

Biography: Dr. Moscoso completed residencies in Internal Medicine and General Surgery in his native Cuenca, Ecuador. After immigrating to the United States in 1990, he completed a Family Medicine Residency at the University of Texas Medical School at Houston, where he served as chief resident. He stayed on as assistant professor at the Department of Family and Community Medicine for several years before joining the Memorial Family Medicine Residency Program. Dr. Moscoso is particularly interested in office procedures and health care policy.

Samuel Wang, M.D.

Biography: Dr. Wang received his medical degree from the University of Texas at Houston and went on to complete a Family Medicine residency at St. Elizabeth Hospital in northern Kentucky. Following his residency training, Dr. Wang practiced medicine in a large multi-specialty group in Houston for over four years before joining the Memorial Family Medicine Residency. He now serves as medical director for the Sugar Land Clinic. Dr. Wang is board certified in Family Medicine and has a special interest in computerized medical records. He is married, has two children and speaks Mandarin. His hobbies include computers and technology, music, racquetball and cars.

N. Perryman Collins, Jr., M.D.

Biography: Dr. Collins was in private practice prior to joining the faculty in 1995. He earned a B.S. degree from Louisiana College in Pineville, LA, and his medical degree from the University of Texas at Houston. He completed his Internal Medicine residency training in 1988 at Tulane Affiliated Hospitals in New Orleans. He is board certified in internal medicine and is the director of the Family Medicine Adult Inpatient service at Memorial Hermann Southwest Hospital. He has been with the Memorial Family Medicine Residency Program for 10 years. He has an interest in research and serves at the co-instructor of the Research Seminar for the Memorial Family Medicine Residency Program. His hobbies include spending time with his wife and two teenagers.

Danuta S. Blicharski, M.D.

Biography: After completing her fellowship, Dr. Blicharski was in private practice in Houston for 20 years. She joined the faculty of the Memorial Family Medicine Residency Program in 1999 as instructor in Pediatrics. She loves teaching residents and to pass on to the next generation all she has learned during her years in practice. She believes that passing on what she has learned, makes those years worth twice the value. She has also kept her interest in Pediatric Hematology and gets excited when she sees an abnormal CBC. Her husband is professor of Psychiatry at Baylor Medical College, where her older daughter is entering her final year of medical school. Her younger daughter is a market analyst in New York City.


Pharmacology

Julie D. Adkison, PharmD

Biography: Dr. Adkison joined the Memorial Family Medicine Residency faculty in April 2002. She received a B.A. degree from the University of Texas and a Doctorate of Pharmacy degree from the University of Houston. She completed her residency with the Memorial Hermann Healthcare System and worked in a large public health system before joining the faculty. Dr. Adkison is responsible for educating residents on the appropriate use of pharmaceutical agents, as well as cost effective prescribing. Her areas of interest include diabetes, complementary and alternative medicine and drug information. She is married and has three young sons.

Paul Locus, M.D.

Biography:After residency, Dr. Locus served in the US Air Force for a combined active duty and reserve service of 11 years.  He served as faculty and then director of the Obstetrics and Gynecology Education Program at Brackenridge Hospital in Austin, Texas, as well as Assistant Director of the St. Joseph's Ob/Gyn Residency Program until 1998.  Throughout this time period Dr. Locus also maintained a private obstetrics and gynecology practice, which has been his sole professional focus until December 2007, when he returned resident teaching.  He enjoys hands-on resident education and practices complete obstetric and gynecologic care, both clinic and inpatient, medicinal and surgical.  Dr. Locus is fluent in American Sign Language and has served on the Texas Commission for the Deaf and Hard of Hearing ( now DARS).

Biography:  After obtaining his undergraduate degree, Dr. Bush worked in a psychiatric setting for several years before pursuing a Master of Arts degree in Clinical Psychology.  He then entered the PhD. Program in Clinical Psychology. As part of his degree program he completed an internship at Baylor College of Medicine in Houston, TX followed by a Post Doctoral Fellowship.  He was a faculty member at Baylor for one year before entering full time private practice. He has remained an adjunct faculty member at Baylor college of Medicine throughout his private practice career.  He has long standing interests in mood disorders, anxiety, personality development, learning disabilities, recovery from head injury, and the treatment of sleep disorders.
